Tick box to indicate that you, hereinafter the "client", having read and understood the Melody House Photoshoot Policy, Terms and Conditions agree as follows,  THAT:

1. The client, an adult/ a person of legal age, desires to take part in a photoshoot offered by Melody House.
2. Client's personal details appear in the PHOTOSHOOT BOOKING FORM
3. The client understands that "photoshoot" means "photography shoot" and that photography shoot refers to taking of still photography and/ or videography by Melody House.
4. The client has agreed to be photographed in a session which shall take place at a time and place offered by Melody House.
5. The client hereby grants Melody House P.O Box 56536, 00200, Nairobi, also trading as Music and Film Academy, its successors, affiliates, associates, agents and students the irrevocable permission to create and distribute client's image and likeness as provided in this document.
6. All photographic materials, such as original negatives, photos, or slides, shall be the exclusive property of Melody House.
7. Melody House shall own the copyright in all images created and may use the work for samples, contests, exhibition, advertising and self-promotion in areas of photography, videography, education, studio, sound, music, style and fashion, beauty and services offered by Melody House and/ or Music and Film Academy.
8. The client accepts and permits, in respect to the photoshoot, the distribution of client's image and likeness by way of publishing, sharing and posting on websites and social media pages in any form and for any period in any part of the world.  Client in addition permits that Melody House may use the assumed name/ artist name/ nickname where provided in the photoshoot booking form to put a name to the photography.
9. Client may be photographed individually or together with other persons at the photoshoot.
10. The client has agreed to take part in the photoshoot on their own free will and at no fee payable to them.
11. Client accepts that all photographs may be taken and edited by Melody House based on Melody House's experience and artistic view.
12. The photoshoot session shall come to an end by either party election or parties mutual agreement. Ending of the photoshoot session however shall not affect party accrued rights or any photography already taken.
13. No obligation shall be placed on either party or a party representative for not attending the photoshoot session.
14. Client shall within 7 days of the photoshoot obtain from Melody House select prints of still photography for personal use only.
15. Melody House shall not be responsible for archiving photography 7 days after the photoshoot.
16. By clicking submit button on this website the client signifies that they have issued written confirmation of their agreement to the Melody House photoshoot policy, terms and conditions herein.
